New Student Registration Check-off List

Please remember to bring the following forms.  Registration Form may be completed on-line, but must be printed, then signed and submitted to the Office of Lifelong Faith Formation.

Turn in all forms and payments to Vince Castellano, Director of Lifelong Faith Formation or Louise Vine, Assistant Director of Lifelong Faith Formation.

___ Student’s Baptismal Certificate: Bring your child’s original Baptismal Certificate with Seal. We will make a copy and return it to you.

___ Parish Registration form –All families must be registered with our parish in order to participate in Faith Formation.  Forms are available in the lobby on Sundays.  Ask an usher if you cannot locate them.

___ Registration Fees:    $45 - 1st student,

$30 – 2nd student,

$25 – 3 students or more.

___ Sacramental Fees:        $25 Reconciliation & First Eucharist, (second grade or older)

        $25 Confirmation - Only incoming 8th graders need pay, not those who began their preparation last year. A Retreat fee will be collected later.
