Returning Student Registration Check-off List

(If your child did not attend Religious Education classes during the 2010-2011 year, you must complete all the paperwork for the New Students.)

Please remember to bring the following forms to the Office of Lifelong Faith Formation.

Turn in all forms and payments to Vince Castellano, Director of Lifelong Faith Formation or Louise Vine, Assistant Director of Lifelong Faith Formation.

___ Student’s Baptismal Certificate: If you have not already submitted your child's Baptismail Certificate, please bring your childs's original Baptismal Certificate with Seal. We will make a copy and return it to you.

___ 2011 – 2012 Lifelong Faith Formation Registration Form (CLICK HERE) (We prefer you bring in the print out of your Family Registration Form you received last spring, if you were not there the last day of school then we will have that packet in the Office of Lifelong Faith Formation, see Louise Vine. If you lost the form, you may go on-line and fill out, print out and sign.)
